Roles and Responsibilities of a Social Worker – Case Manager: 

Client Assessment: 

Conduct thorough evaluations to understand the client’s needs, strengths, and challenges, encompassing their physical, emotional, social, and environmental situations. 

Individualized Care Planning: 

Design and implement tailored care plans that address the unique needs of each client, including goal setting, service identification, and service delivery methods. 

Service Coordination: 

Serve as the primary liaison between clients and necessary services or resources coordinating with healthcare providers, counselors, community groups, and other relevant entities.
